Important: Questionnaires must be postmarked or submitted no later than June 8, 2026.
To submit your Claimant Questionnaire, please login with the Unique ID and PIN printed on the Notice that was sent to you. If you lost or did not receive a Unique ID and PIN but believe you are a Claimant, you should contact the Settlement Administrator at info@EEOC-EnterpriseSettlement.com to request a Unique ID and PIN.

The deadline to submit a Claimant Questionnaire online or by email is June 8, 2026. If you are mailing your Claimant Questionnaire, it must be mailed with a postmark date no later than June 8, 2026.
